Purchase your tickets for the PTHS Winter Glow Dance!!

EVENT DATE: Saturday, February 25, 2023

LOCATION: PTHS Cafeteria

ARRIVAL TIME: 7:00-7:30PM (No admittance after 7:30pm)

END TIME: 10:00PM (Any student/guest leaving before 10pm will not be readmitted)

TICKET PRICE: $15 for early registration by 2/23 ($20 at the door)

PTHS Students may bring one (1) non-PTHS guest. All attendees must submit the required forms below in order to attend. All guests must be UNDER 21 years of age.

PTHS STUDENTS INSTRUCTIONS:

1 - Download the Student Waiver Form

2 - Complete all required information including parent signature

3 - Email the form to the following email address with the subject line "FORMS FOR WINTER DANCE": pthswinterdance@gmail.com

4 - Deadline to submit forms online: Friday, February 24th!

Non-PTHS STUDENT GUESTS INSTRUCTIONS:

1 - Download BOTH the Guest Registration Form AND the Student Waiver

2 - Guests must complete the forms (including parent signature & their school admin approval)

3 - Email BOTH forms to the following email address with the subject line "FORMS FOR WINTER DANCE": pthswinterdance@gmail.com

4 - Deadline to submit forms online: Friday, February 24th!

If unable to make the form submission deadline, be sure to bring the completed forms with all signatures to the dance to gain entry. No student will be admitted without these completed forms.

Light snacks and water will be provided. The event will be chaperoned by parent volunteers and there will be PT Police Officers in attendance.
